Off-Reserve - Thanksgiving Grocery Gift Card Application

This application is for off-reserve registered members applying for a Grocery Gift Card that they can use towards their Thanksgiving Meal.

On-reserve households will be receiving gift cards through Prevention, so this application is for off-reserve households only.

Important Information:

One grocery gift card per household.
Proof of address is required (if you previously send proof of address and have not moved you do not need to send it again)
Applicant must be a registered Fishing Lake First Nation member.
